Bias and ground truth?

Unless I am mistaken, determining bias involves measuring a distance to a ground truth.
If correct: what is the ground truth in this experiment? How do we know whether there IS a bias, or not?
Could it be considered absolute, or is it a function of, say, time, geography, culture, beliefs, or even language?
If there is such an absolute ground truth: what is it, who defined it and how, how can we measure it, and optimise to reduce it?
